Mainstay Physical Therapy accepts Medicare, Medicaid, Aetna, and Cigna plans at this time and also offers a cash-pay option with rates as follows:

Initial evaluation: $100 for 1-hour appointment.
Follow-up visit: $50 for 30 minute appointments.
$70 for 45 minute appointments.

Mainstay accepts cash, check, credit, and FSA/HSA.

Biography


Jessica Malouf PT, DPT is so excited to be opening the Greeley branch of Mainstay Physical Therapy at Ancient Wisdom Healing Arts! Jessica opened Mainstay Physical Therapy in April of 2016 in Fort Collins, CO with the mission to provide convenient, effective, and highly skilled rehabilitation for people with:

Dizziness and vertigo
Headaches and migraine
Balance impairments
Neck and back pain
Myofascial dysfunction such as fibromyalgia or myofascial trigger points
Neurologic conditions and mobility impairments
Orthopedic injuries
Post-surgical rehab
Chronic pain conditions
Facial paralsys

Jessica received her Doctor of Physical Therapy from The University of Montana in 2008 and moved to Colorado from Missoula, MT in 2014. She has earned the vestibular rehabilitation competency for both children and adults through the American Physical Therapy Association, Emory University, and Specialty Therapy Source. She is also a certified Cognitive Behavioral Therapy life coach.

Treatments at Mainstay Physical Therapy include vestibular rehabilitation, trigger point dry needling (TPDN), instrument assisted soft tissue mobilization (IASTM), joint mobilization, movement retraining, specific therapeutic exercises, skilled relaxation training, neuromuscular and transcutaneous electrical stimulation, proprioceptive neuromuscular facilitation (PNF), posture retraining, graded motor imagery, and cognitive behavioral therapy techniques to allow clients to achieve enhanced mobility, reduced pain, improved balance, reduced or resolved dizziness and vertigo, and improved confidence in their ability to manage their chronic conditions successfully.
